Position Summary: Contracted PT/PRN Positions

 

MUST HAVE ACTIVE NPI AND FLORIDA MEDICAID PROVIDER NUMBER

 

Completes comprehensive assessment of clients’ substance abuse history and treatment requirements, and develops individualized treatment plans.

 

Serves as a the licensed evaluator and supervising provider for counseling services.

Reviews treatment plans and provides an overview to counselors

 

The provider will be expected to help generate referrals for the business and service clients of the business. Willingness to serve as billing provider for organization and review notes of counseling staff and co sign treatment plans.

​

The evaluator must be able to accurately diagnosis clients of mental health or behavioral health conditions. Must also be able to accurately update diagnoses as symptoms and/or behaviors changes.  

 

QUALIFYING WORK EXPERIENCE: 

 

A Master's degree in a mental health field, Social Work, Psychology, Counseling or any human service related field, from an accredited college/university. Must have active Florida Medicaid Billing Number OR be eligible in the state of Florida and have an NPI
