Position Summary: Contracted Position/PRN

 

MUST HAVE ACTIVE NPI AND FLORIDA MEDICAID PROVIDER NUMBER

 

Completes comprehensive assessment of clients’ substance abuse history and treatment requirements, and develops individualized treatment plans.

 

Serves as a the licensed evaluator and supervising provider for counseling services.

Reviews treatment plans and provide overview to counselors

 

Documents treatment and discharge plans and clients’ progress and response to treatment and maintains related records and files.

 

May provide  individual and group counseling, education programs in accordance with treatment plan. Documents treatment and discharge plans and clients’ progress and response to treatment and maintains related records and files. Complete documentation related to individual, group, and family sessions that are provided to the youth, as well as, monthly progress updates related to mental health services.

 

Understanding of the requirements of supervision, including meeting individual and program goals, working with different counselor styles, initiating and completing the disciplinary process (as necessary), organizing treatment efforts/treatment delivery; 

 

QUALIFYING WORK EXPERIENCE: 

 

A Master's degree in a mental health field, Social Work, Psychology, Counseling or any human service related field, from an accredited college/university. Preferred that the staff member is a Licensed Mental Health Counselor in the state of Florida and have an NPI and Medicaid Number.

Internships Positions Available Immediately:

Marketing/Business Development Internship

Description:

Perform targeted marketing efforts to identified referring entities to to cultivate the referral relationships and to build a committed referral partner for OHO. Attend community events that provide an opportunity to both identify potential top referral sources and to maintain existing referral relationships

Principal Duties and Responsibilities:

                         

• Drives business and market growth for counseling and case management service lines.

 
• Ensures confidentiality of OHO information, documentation, and records as required.

 
• Knowledge of at risk communities and relationships with community advocates and leaders is required .


• Perform field work by visiting pediatrician, family provider, psychiatrists, churches, community events, to generate referrals for OHO and partnering organization. 


• Connect with community leaders to establish relationships on behalf of OHO to generate referrals for organization and maintain resource directory for OHO and partnering organization. 


• Utilize social medical to increase awareness of OHO services.

 
• Develop relationships with community partners to promote the services of OHO,LLC for referral linkage and establishment of resources for OHO and partnering organization.

 
• Attend community and networking events as requested required.

Social Work/Targeted Case Manager Internship

 

Description:

Optimum Health Outcomes is interested in obtaining interns to assist with providing services to our clients. 
Responsibilities: Students have the opportunity to work in the community to develop referrals for OHO and partnering agency, develop resource directory, network with community, healthcare and behavior health providers. 
Develop curriculum for internship program, manage OHO social media resources and perform assessments, ISP and progress notes for clients assigned. 

The interns will assist the clients in obtaining services in the community and keeping those services. The means for working with clients is defined through the clients treatment and identified goals. Most work is done in the community with appointments occurring in the clients or with community providers. The student will receive weekly supervision. Driving and transporting clients is a possibility. 

Accepting 2 positions for the Internship. Interns must be willing to see clients as assigned. Mileage will be reimbursed. Internship is unpaid with a stipend to be provided at the completion of three months.

 

 

Responsibilities:

  • Develops progress notes, client assessments, individualized service plans to deliver quality services in accordance with AHCA guidelines.

 

  • Conducts weekly marketing calls as directed.

 

  • Establish and maintain referral sources and contacts.

 

  • Develops new referral sources.

 

  • Provide customer service.

 

  • Maintains individual account status records for each. 

 

  • Maintain strict confidentiality of clients

 

 

 

Requirements:

College Senior with coursework completed in psychology, marketing, social services and business. Master degree seeking students in Psychology, Marketing, Health Science, Rehabilitative Services, Criminology or Social work.
