Optimum Health Outcomes, L.LC – Orange, Brevard, Lake, Seminole and Osceola County
Positions Available Immediately:
We are looking for a Case Managers who can build their own case load and bring referrals to the organization. The successful candidate will be accountable for building their own case load and adhering to Medicaid guidelines. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a plus.
Minimum Required Qualifications:
Bachelor's degree or higher (with major course work in sociology, social work, psychology, gerontology, , criminal justice, nursing counseling or education)
OR
Bachelor's Degree or higher a non-human services related field AND at least 2 years of working experience or equivalent experience working with individuals with mental health needs

Bilingual English/Haitian Creole/ Spanish preferred

Demonstrate ability to communicate effectively, both verbally and in writing.

Able to pass child abuse, state police and FBI clearances (Level 2).

Possess a valid driver’s license and have daily access to a privately maintained vehicle

Successful completion of AHCA-approved mental health targeted case management training 90 days of initially providing Medicaid services
Job Function and Responsibilities

Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered

Refer clients to the organization and build a client case load Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered

Manage clients, develop service plans, assessments, review case progress and recommendations for case closure

Help clients achieve wellness and autonomy

Manage clients, develop service plans, assessments, review case progress and recommendations for case closure

Facilitate multiple care aspects (case coordination, information sharing, etc)

Complete accurately all necessary forms and documents for client services to administration to adhere to Medicaid guidelines
Skills
-
Excellent knowledge of sources to obtain clients for maintaining case load and referring clients to the organization
-
Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
-
Excellent knowledge of case management principles, healthcare management and reimbursement
-
Previous experience with psychological aspects of care
-
Effective communication skills
-
Problem-solving skills and ability to multi-task

Facilitate multiple care aspects (case coordination, information sharing, etc)
